Human Services Navigator

Schedule: Monday-Friday 8:30am-4:30pm, with frequent weekend and evening outreach events

Position Summary
The Human Services Navigator is a full-time position providing screening, information, referral, and intensive case management services to vulnerable individuals and families, liaison services to community providers, system partners, Navigator reports to the Human Services Planning Administrator. Knowledge of safety net programs and services, principles of human services case management and human services systems, is essential to performing this position. The Navigator will work with clients, primarily on location, at Foodbanks located throughout Somerset County.
The Navigator builds inter-system relationships and serves as a resource of system partners and the community at large; Navigator will serve as a point-of-contact for individuals who live or work in Somerset County in need of accessing an often-complex human service support system.

Essential Functions 

  • Provide information, referral and case management to residents across the lifespan, who live or work in Somerset County, with human service needs.
  • Provide follow-up and warm-off to the residents indicated above.
  • Provide case management services to residents identified as needing a higher level of support connecting to safety net services and resources.
  • Attend monthly councils, meetings, and events; ongoing outreach opportunities within various sectors of the community, including but not limited to Libraries, Churches, Food banks, Hospitals etc. 
  • Research and collet data to identify community needs, available services and how individuals’ access services;
  • Meets with supervisor (Planning Administrator) regularly to provide supervisor updates and feedback. Additional support from the Mental Health Administrator as needed.
  • Performs all functions pertaining to the Navigator operations including office administration tasks to adhere to County and State requirements.
  • Community presentations with a focus on (but not limited to) navigating human service support systems and the supports available to assist residents of the County
  • When not directly assisting identified resident inquiries, assisting other Human Services initiatives or staff.  
  • Perform other duties as assigned by management

Essential Qualifications

  • Bachelor’s degree in human services, Counseling, Social Work, Public Administration, Public Health or related degree with a background in substance use disorders and/or co-occurring disorders. (work experience may be substituted for education).
  • At least 2 years’ experience in the field of Human Services.
  • Knowledge of safety net programs and support systems.
  • Knowledge of strength-based approaches.
  • Proficient in use of computer for data entry and analysis.
  • Excellent time management, analytical and organizational skills.
  • Advanced verbal, written and analytical skills.
  • Comfortability with public speaking; ability to work with cultural humility
  • Ability to work a flexible schedule of hours as needed (including weekends and evenings as needed)
  • Valid & current New Jersey Driver’s License.
  • Criminal background and credential verifications must pass Somerset County standards

Preferred Qualifications 

  • Master’s Degree in Social Work or human services related field
  • Bilingual in Spanish
